Great Rivers - Hiring a Community Health Worker

Learn how a group of committed individuals can impact real change in your community! From doing resource mapping to leading families through research driven pathways that get at the root cause of their challenges, you'll hear success stories from our Community Health Workers that were otherwise un-achievable in the traditional public education service model.

We will share the foundational work, which led to the development of the Jackson County Servant Led Community. This group explored ways to maximize the resources the County had in place, identify priority resources to further develop, and find ways to build connections between those in need and the available resources. This work brought us to the Pathways Community Hub model and the development of Community Health Workers within Jackson County, specifically in the School District of Black River Falls. When all agencies recognize that the schools have the greatest access to families and begin to push resources into our schools as opposed to standing separately, you can accomplish great things!

PRESENTER: Shelly Severson, Superintendent - Black River Falls School District

Shelly Severson has served the Black River Falls Community as superintendent for the past eleven years. She serves as a director on several community boards and is a founding partner of the Jackson County Servant Led Community. Shelly seeks new ways to serve the students and families within her community and developed strong partnerships in order to ensure they are working as efficiently and effectively as possible in meeting the needs of Jackson County residents.

PRESENTER: Lindsey Purl, Director of Great Rivers Hub

Lindsey Purl was hired a Director of Great Rivers Hub, a Pathways Community HUB model, in April 2017. Lindsey earned her Master’s Degree in Public Health-Health Policy and Administration from the University of Illinois at Chicago in 2017. She earned a bachelor’s degree in Community Health Education and Exercise Sport Science from the University of Wisconsin-La Crosse in 2011. Prior to her role as the Director for Great Rivers HUB, she has an extensive background working with at- risk populations and in non-profit program development.

PRESENTER: Leslie Ruffalo, Medical College of Wisconsin

Leslie Ruffalo, PhD MS is an Associate Professor in the Department of Family and Community Medicine at the Medical College of Wisconsin. Dr. Ruffalo serves as the Director of Medical Student Education in her department and is a faculty member in the Office of Community Engagement. Dr. Ruffalo received her undergraduate degree from the University of Wisconsin-Madison and her Master’s and Doctorate degrees from the University of Wisconsin-Milwaukee. Dr. Ruffalo’s research interests broadly span health equity topics through the methods of community engaged research. Her primary research interests include rural health, school-based health and wellness, and the use of community health workers to enhance health and wellbeing.
